Registration Requirements

Before You Register:

USA Ultimate Membership: All players must have an active USA Ultimate Youth or Affiliate membership and liability waiver.
Medical Form: Complete the USAUltimate Medical Release form.
Join the Mailing List: https://groups.google.com/g/fairview-hs-ultimate-frisbee (put player name as reason for joining).

Please complete all fields in the registration form below. Both the medical form and an active USA Ultimate membership are required to participate in practices and games.

Team Fees & Payments

After completing your registration, please submit your payment via RevTrak, link below. For financial assistance, contact fairviewultimatefrisbee@gmail.com

Season Dues

Pay your season dues securely through RevTrak. Link goes directly to the BVSD payment portal.

Coaches Fund

Coaches cannot be paid from RevTrak fees. We try to collect funds to compensate them from families able to help out via Venmo (@ejfrankel).
